Website privacy practices
This policy explains how information you share through this website is handled. It applies to the website only. The privacy of your clinical records and care is governed separately by HIPAA and the practice's Notice of Privacy Practices, which I'll provide before treatment begins.
What this website collects
If you choose to use the contact or insurance-verification forms, we collect only the information you enter, such as your name, email, phone number, preferred contact method, insurance provider, and any message you write. The forms intentionally do not ask for sensitive medical details, member ID numbers, dates of birth, or Social Security numbers.
How form submissions are handled
Form submissions are delivered by a third-party form service (Web3Forms) and sent by email to me. Because this on-site form pathway is not a HIPAA-secured channel, please keep what you share general. I follow up privately through a secure, HIPAA-compliant email account to discuss anything personal and to verify insurance benefits.
How your information is used
Information you submit is used only to respond to your inquiry, verify benefits if requested, and schedule services. It is not sold or shared for marketing.
Analytics & cookies
This site aims to keep tracking to a minimum. If basic, privacy-respecting analytics are used, they are limited to understanding general site usage and are not used to identify you personally.
Not for emergencies
This website is not monitored for emergencies and does not provide crisis services. If you are in crisis, call 911 or call/text 988, or see our crisis resources. Using this website does not create a therapist–client relationship.
